Vision from the scene shows paramedics working on the girl as she was loaded into the ambulance. She was taken to Sydney Children's Hospital at Randwick but later died.

Canterbury Rd was closed in both directions following the incident.

A crime scene has been set up and the Metropolitan Crash Investigation Unit will be investigating.

The 43-year-old female driver has been taken to hospital for mandatory blood and urine tests and is assisting investigators.

A report will be prepared for the Coroner.

Campsie Police Superintendant Adam Whyte said the incident was an "absolute tragedy" especially with Christmas around the corner.

"Police and ambulance were confronted with a horrific scene where they administered immediate first aid to the child who was critically injured," Supt Whyte said.

"The female driver of the vehicle has been conveyed to hospital for mandatory testing. She is hysterical.

"It's a reminder to all drivers who are reversing vehicles at busy times such as Christmas to make sure they are vigilant in checking what's behind the vehicle."
